SITE-VISIT AGREEMENT (Draft)

This AGREEMENT is made on (DATE) between Yosemite Close-Up LLC, 5109 Jessie St., Box 2312, Mariposa, CA 95338 (CONTRACTOR) and YOUR NAME AND ADDRESS (CLIENT).

1. Services to Be Performed. Contractor agrees to perform the following services for Client: Visit Client’s site at address to be specified; Provide up to five (5) hours of on-site consultation on Client’s property.

2. Time for Performance. Client agrees to complete the performance of these services on or before a specified date (Month, Day, Year).

3. Payment. In consideration of the Contractor’s performance of these services, Client agrees to pay Contractor as follows: Client will pay Contractor $500.00 plus $1.00 per one-way mile from Mariposa Post Office to Client’s address, according to Google Maps’ calculations. Payment(s) will be made by a mutually agreed upon time and method.

4. Invoices. Contractor will submit Invoices for all subsequent services performed.

5. Independent Contractor. The parties intend Contractor to be an independent contractor in the performance of these services. Both Contractor and Client shall mutually determine the method and means of performing the above services.

6. Other Clients. Contractor reserves the right to perform services for other clients.

7. Assistants. Contractor, at Contractor’s expense, may employ such assistants as Contractor deems appropriate to carry out this agreement. Contractor shall be responsible for paying such assistants, as well as any expense attributable to such assistants, including income taxes, unemployment insurance, and Social Security taxes, and will maintain workers’ compensation insurance for such employees.

8. Equipment and Supplies. Contractor, at Contractor’s own expense, will provide all equipment, tools, and supplies necessary to perform the above services, and will be responsible for all other expenses required for the performance of those services.
